I am using Vix big mask 001
you can download from her site

Font
I used one called 
Trevadel Mar Regular

Plug ins
Xero-Grey scaler and mistifier

Lets begin

open a new canvas 700x700 fill with white

add a new raster layer and select all
open paper 5, copy and paste into selection, select none
apply mask and merge group

open frame 2, copy and paste as new layer
place in center of canvas

open your tube, copy and paste as new layer
place in center of the frame and drop shadow
I used -2 -4 70 10.00

open flower 3
paste on left side of tag, and drop shadow
duplicate and mirror

open frame 1
place on right top side of tag 
above your mask layer
click inside of your frame, selections select all
expand by 5, open your tube mirror it
apply xero grey scaler default settings
place what you want showing in the frame
selections invert and hit delete on your keyboard
select none
and drop shadow the frame

open tag, and place on lower right of tag
above your mask layer
and drop shadow

open violin, resize 45%
place on left side of tag
and drop shadow

open bag, resize 40%
place on left side of tag, below violin layer
and drop shadow

open flower 2, resize 45%
rotate left 90, place on left side of tag
and drop shadow
duplicate and mirror

open bottle, resize 40%
place on right side of tag
and drop shadow

open button, place on left side of tag
above mask layer at the top
and drop shadow

open necklace, resize 45%
place on right side of tag, below the bottle layer
and drop shadow

open pillow 1, resize 30%
place on right side of tag, below the necklace layer
and drop shadow

open glitter, rotate left 90
place on right side of tag
above your mask layer

open key, resize 40%
rotate left 18
place next to violin
and drop shadow

open word art, resize 35%
place on left side of tag
and drop shadow

resize and crop your tag
merge visible, go to xero mistifier
apply default settings, and sharpen

apply your artist copyright and your name
save your tag
