I am using 2 fall mask by Dee
DD_Mask_8 and DD_Mask_6

Plug ins
Xero-porcelain

Lets begin

open a new canvas 700x700 fill with white

add a new raster layer and select all
open paper 9, resize 30%
copy and paste into selection and deselect
open dd mask 8 and apply, merge group
move the mask toward the top of your canvas
add a new layer above this mask layer and select all
open paper 1, copy and past into selection and deselect
apply DD mask 6 and merge group, move down just slightly

open frame 3, copy and paste as new layer
rotate 90 left, place in center of your canvas for now
grab your magic wand, click inside all 3 frames
remember to hold down shift key when selecting more than one frame
selections-modify-expand by 8
add a new raster layer move below the frame layer
open paper 6, copy and paste into selection, keep selected
with your frame layer active, open your tube and resize, I resized 55% and mirror
copy and paste as new layer, place what you want showing in the frames
selections, invert and hit delete on your keyboard, deselect
move your tube below the frame layer and duplicate your tube
on the bottom original tube ajust, blur, guassian blur 6.00
on the top copy change blend mode to hardlight 
on the top frame layer give it a drop shadow, I used -2, -4. 70, 10.00
close off your background layer and both mask layers and merge visible
and unhide your layers on your frame layer, rotate right 10
move your frame over to the right side of your canvas

open your main tube, remember to unmirror it
I resized mine 45%, copy and paste to the left of your frame
and apply xero porcelain, default settings 
and drop shadow

open leaf 4, resize 70%
place at the bottom of your tube layer
and drop shadow

open leaf 16, resize 70% and mirror
place on right side of tag, above the frame layer
and drop shadow

open the dog, resize 40%
place in the center above your tube layer
and drop shadow

open cluster 7, resize 40%
place on right side of tag, above frame layer 
and drop shadow

open cluster 3, and mirror
place on right side of tag, below your frame layer
and drop shadow

open leaf 5, resize 40% and mirror
place on right side of tag, center of cluster 3
and drop shadow

open the sign, resize 75%
place on left side below your tube layer
and drop shadow
I used my eraser tool, to erase the chain

open flower 13, rotate left 90
place behind the sign and to the left of tag
and drop shadow

open flower 20, resize 45%
place to the left above the sign layer
and drop shadow

open cluster 6, resize 70%
place on left side of tag
and drop shadow

open cluster 2, resize 50%
place on left side of the pup
and drop shadow

open leaf 6, resize 35%
rotate right 90
place as top layer in center
and drop shadow

open acorn, resize 35%
rotate right 90
place as top layer and drop shadow

open flower 15, resize 50%
rotate left 90
place on left side of tag
and drop shadow

open fruit 5, resize 45%
place behind the pup to the right
and drop shadow

open pinecone, resize 20%
place at top layer to the left of your tag
duplicate and resize 70% and move over to right slightly
drop shadow both

open petals, resize 25%
place to the right of the pinecones
and drop shadow

open bird 2, resize 20%
place above the sign layer
and drop shadow

open butterfly 1, resize 17%
place on right side of tag above frame layer
duplicate and resize 70%, move down slightly
drop shadow both

delete your white background
merge, resize and crop your tag

add your artist copyright
add your name
save
